Black Community Outreach/Aging & Disability Resource Network Specialist
The individual selected for this position will provide outreach to the Snohomish County Black Community defined as African American, African Immigrant, and anyone identifying as Black. Ideally, the preferred candidate will have professional experience working with these communities and have an understanding of the dynamics of various ethnic groups within the Black community. Additionally, the selected candidate will staff the information and assistance phone line several days a week, taking calls and linking older adults and people with disabilities to community resources intended to support them to remain living independently in their home. The division of responsibilities for this position is 60% Aging & Disability Resource Specialist and 40% Black Community Outreach
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provide detailed information on available services, programs and benefits to older adults and people with disabilities, directing the caller to the most appropriate service or program. When needed, facilitate referrals to services or programs using a resource database
- Manage a rotating client list of 30-35 assist cases and meet monthly performance goals
- Maintain accurate and complete records per program and agency standards
- Be a major player in the development and implementation of an outreach program to older adults and people with disabilities of the Black Community
- Build and sustain a group focused on older adults with the goal of meeting regularly and becoming a hub for the Black community
- Meet individually with clients within this group to assist them with connecting to resources and providing necessary follow up
OTHER DUTIES
- Assist with the development and translation of resource handouts and other program materials
- Participate in training designed to improve skills and expand areas of expertise
- Other duties as assigned
EDUCATION AND EXPERIENCE
- AA degree in human services, one of the behavioral sciences or related field (required) and two years of paid experience providing direct human services preferred (minimum 1 year experience required)
- Proficiency using computers running Microsoft Office, confident learning new databases/systems and technically literate
- Previous formal or informal experience providing service to Black communities preferred
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
- Organized, self-motivated, critical thinking skills, flexible, team player, sense of humor
- Must be able to communicate, read and write proficiently, in English; bi-lingual a plus
- Comfortable conducting outreach and making presentations required; experience preferred
- Be AIRS certified or obtain AIRS certification at the one year anniversary of employment
- A valid WA drivers license and current auto insurance required
Work is performed primarily in an office environment. The office environment may include a shared workspace and is fast-paced with frequent interruptions. The functions of this position are not necessarily physically demanding. The functions are usually performed sitting, but will require some amount of time standing, as well as occasional stooping, climbing stairs, bending, crouching, lifting, walking, carrying and other movements may be required.
